Causative Agents
Organism is a small, non-motile, Gm-ve bacilli Four Medically important species are present, B abortus, melitensis, suis & canus. Successful culture, in only 20% of acute cases It needs special media for cultivation, blood agar + 10% Co2. Subspecies are host adapted. Blood cultures or bone marrow, lymph node, or spelnic puncture may be useful in chronic cases, taken during chills. Fortunately, isolation of brucellae decreased by refrigeration,Prof JAMAL R Al-RAWI-MBChB, MSc, FICMS-2012
EPIDEMIOLOGY
World-wide endemic diseaseHighly endemic in Mediterranean region (hyper endemic in IRAQ Saudi Arabia .Less common in other civilized countries like European, American, Australian & JapanEradicated in theses countries, because of the control of diseases in animals (human cases were positively correlated with animal cases). In USA 200 cases/year reported Ѕ of them occupational (Slaughter-house).Occupational brucellosis happen to those at high risk in contact with animals; butchers, veterinarian, animal house-keepers, cow-boys, farmers etc… Prof JAMAL R Al-RAWI-MBChB, MSc, FICMS-2012EPIDEMIOLOGY- Continued
In In Al-Aska, countries 2-3% of the cases happened among veterinarian during processing of live- vaccines. USA the commonest type is abortus, this related to the big companies that produce meat & milk & its products. In European countries, suis is the commonest because of the pigs consumption compared with other countries. In Mediterranean melitensis is the most common types. Seasonal variation with peak incidence in April-May.Prof JAMAL R Al-RAWI-MBChB, MSc, FICMS-2012
Mode of transmission Domestic animals is the reservoir Animal to animal transmission ANIAML TO HUMAN TRANSMISSION HUMAN TO HUMAN. Occupational brucellosis happen to those at high risk in contact with animals; butchers, veterinarian, animal house-keepers, cow-boys, farmers etc..
